Family
…am expecting or had a baby?
  • Notify your manager if you will be absent from work due to your child’s arrival. Discuss your plans for:
    • Salary Continuation (also known as Short Term Disability)
    • FMLA Medical Leave
    • Parental Leave (if non-birth parent)
    • Paid Time Off
  • Use ALEX: Leave of Absence to get the support you need to understand how pay and benefits work while you’re on leave.
  • If your spouse is giving birth, you may consider taking additional leave under FMLA, which includes time to bond and care for your new child.
  • If you or a spouse is pregnant and covered under a Medtronic Medical Plan, sign up for the Healthy Pregnancy Program through your medical plan administrator. This program includes tools, resources, and nurse support designed to help participants have the healthiest pregnancy possible. When you or your covered spouse completes the program, you will be offered an additional two weeks of paid parental time off. For details about the Healthy Pregnancy Program, call your medical plan administrator.
  • If your baby is born between Jan. 1 ‒ May 31, 2018, you must complete two of the three components of the Healthy Pregnancy Program, including telephonic nurse support and a post-partum assessment.
  • To apply for salary continuation, FMLA or parental leave, contact Matrix Absence Management at (888) 477-5110 no earlier than 30 days prior to your due date, or as soon as you begin missing work.

  • You must enroll your child within 31 days of his/her birth to receive coverage. You’ll enroll online through Workday. Once you have your child’s Social Security number, enter it in Workday.
  • If you don’t enroll your child within 31 days, your next opportunity to enroll him/her is during open enrollment for elections that take effect as of January 1 of the following calendar year.

  • Notify your manager if you will be absent from work due to your child’s arrival. Discuss your plans for:
    • FMLA Medical Leave
    • Parental Leave / Adoption Leave
    • Paid Time Off
  • Use ALEX: Leave of Absence to get the support you need to understand how pay and benefits work while you’re on leave.
  • If you adopt a child or have a child through surrogacy, you are eligible for two weeks of paid parental time off.
  • If you are adopting a child, you may be eligible for adoption assistance.

  • Notify your supervisor or manager (or if you’re a survivor, the employee’s supervisor or manager) of the death and any arrangements.
  • Contact the Medtronic EAP (Company ID: Medtronic) for confidential and no cost support, consultation, resources and referrals for grief and loss, or other needs you or your family may have.
  • Review the Bereavement Leave Policy on the Medtronic Policy Portal.
  • If you need to take a leave of absence beyond bereavement leave, contact the Disability/Absence Operations Team at:

  • If applicable, notify the family member’s employer or school of the death and to determine if there are any continued benefits.
  • If applicable, contact your local Department of Motor Vehicles (DMV) for motor vehicle titles, as well as the bank, creditors and governmental agencies to update names on accounts.

  • For a child turning 26: Your child will be automatically dropped from benefits the 1st of the month following his/her 26th birthday. A COBRA packet will be sent from WageWorks shortly after with details about your options to continue coverage.

Note: Covering ineligible dependents is a violation of Medtronic policy and could result in termination and/or the requirement that you pay back claims. You should take the opportunity to remove any ineligible dependents from your Medtronic benefits immediately after they lose eligibility.

  • Your eligible dependents may apply for COBRA continuation of health coverage within 60 days after becoming ineligible.
  • Make changes to your 401(k) and related beneficiary designation online at the Medtronic retirement website or by calling the Retirement Service Center at (844) 335-9042.
  • Update your life insurance beneficiary designations online through Workday.
  • Renew and update your Federal W4 and State income tax withholding information as needed on Workday.

Certain benefits can only be changed if you experience a qualified status change or during open enrollment, while others are open to changes throughout the year. Prior to making a change to your benefits:

  • Make sure you understand whether you can make changes to the benefit you are looking to change.
  • If you’ve experienced a qualified status change, you must make changes to your benefits within 31 days of the date of your change in status.
  • Visit Workday to make changes to your benefits, and the Medtronic retirement website to make changes to your retirement benefits.
  • Once you’ve made changes, be sure to print a confirmation for your records.

  • Notify your manager if you will be absent from work to care for yourself or your immediate family member. Discuss your plans for:

  • If your sickness or injury lasts longer than 26 weeks, you may be eligible for long-term disability benefits that replace a portion of your income.
  • In the event your leave of absence is not covered by FMLA or you need to remain on medical leave after FMLA is exhausted and the sickness or injury prohibits ability to perform the essential job functions, you can request an accommodation to perform your job.

Health, life insurance and disability:

You have 31 days from your date of hire or change in eligibility/status change to enroll via Workday

Retirement:

  • Unless you elect otherwise, you’ll be automatically enrolled after 60 days of eligible employment and 6% of your eligible pay will be deducted on a pre-tax basis.
  • Your contributions will automatically increase by 1% each year until you reach a contribution rate of 10%. Your contributions and the company match will be invested in the Vanguard Target Retirement fund designed for someone your age who intends to retire at 65. You can enroll earlier, opt out of automatic enrollment and escalation or change your contribution rate and investment elections at any time.
